Incident Investigation Course
2 days – NQF Level 5
Incidents can be prevented. Incidents cost money. Incidents affect people’s quality of lives permanently and negatively.
Through applying effective incident investigation principles it is possible to prevent similar incidents from happening again.
It is not easy to work with witnesses, the different sources of information and to get to a fair outcome. The focus should be on finding the facts and not finding fault.

- Newly appointed incident investigators
- Health and Safety representatives
- Managers and investigation team members
- Established incident investigators who needs a refresher
Pre-Requisites
The following courses should successfully be completed before attempting the Incident Investigation course:
- Occupational Health and Safety Act or,
- Mine Health and Safety Act Overview courses
- Hazard Identification & Risk Assessment (HIRA) course
- Health and Safety Representative course (recommendation)
Course Objectives
To equip delegates with the knowledge and skills required to fulfill legal compliance requirements and participate in, or lead an incident investigation process in the workplace.
They will be able to collect evidence, identify the basic and root causes of incidents, document findings and complete necessary records and legal compliance reports. Delegates will be able to make recommendations to prevent future recurrences.
Course Outcomes
At the end of this course delegates will be able to:
- Discuss incident investigation terminology
- Identify the difference between cause and effect
- Apply the principles of control and ISSMEC principle
- Differentiation between unsafe acts and conditions
- Discuss the Frank Bird ratio
- Identify the pre-event assessment: probability and consequence
- Identify what is needed – kit preparation
- Discuss the pre-event planning requirements
- Classify incidents, evaluate and document findings
- Differentiate between immediate and root cause analysis
- Identify risks – weighting, control methods and costing
- Apply the “Four T’s” and identify applicable legislation
Learning Material
Delegates will receive the Incident Investigation course manual and applicable Investigation forms.
Language
This course is presented in the English language. By prior arrangement, on-site courses for groups of ten or more delegates can be presented in African Languages.
Course Requirements
It is advisable to have prior health and safety knowledge and skills within an organizational environment before attempting this course.
It is always much more effective if we use your own company documentation during training. It is suggested that you bring your incident investigation appointment letter; incident investigation documentation and any other related documentation to this training session.
Certification & Assignment
You will be issued with a Certificate of Competence on successful completion of this course.
